Southampton based Cunard has been ordered to tighten safety procedures after a scalding accident on its luxury liner Queen Elizabeth 2 led to the death of a crew member.

Edgar Villasis died after he and colleague Nelson Venzal were covered in hot water and steam while working on the removal of boiler mountings.

Both were treated on board the vessel as it headed for Southampton from New York on June 23 last year.

When the vessel came within helicopter range, 250 miles south-west of Ireland, the Filipino ratings were airlifted to hospital but Mr Villasis later died.

A Marine Accident Investigation Branch report said hot water and steam unexpectedly discharged from the port boiler main steam stop valve.

The report said a later inspection found a leaking valve which might have led to the build-up of condensation which resulted in the accident.

A Cunard spokesman said: "The system had been statutorily inspected on a regular basis for a number of years and had satisfied all requirements.

"However, recommendations have now been implemented or are being implemented.''
