A Hampshire sex offender branded a "monster" died behind bars after failing to receive consistent care, an investigation has found.

The Prisons and Probation Ombudsman (PPO) has published a report into the death of Roy Jacobs, who was jailed for historic offences against two young girls.

Jacobs, 77, of Orchard Way, Dibden Purlieu, died on September 27 2020 after suffering a "catastrophic" stroke at Winchester Prison.

The PPO report says he suffered from chronic kidney disease, raised blood pressure, cardiovascular disease, and chronic obstructive pulmonary disease (COPD).

It adds: "There is no evidence of a care plan appropriate to Mr Jacobs' medical conditions, resulting in inconsistent care.

"The Head of Healthcare should review the policy on the use of a care planning approach for patients with chronic diseases and ensure that care plans are formulated and followed by clinical staff."

Jacobs died 13 months after standing trial at Southampton Crown Court, which was told he was in poor health and likely to die in prison.

The court also heard from a relative of one of his victims, who described him as a "monster".

Jacobs was jailed for 18 years after being found guilty of sexually abusing one girl between the late 1990s and early 2000s. He admitted abusing another girl in the 1970s.

Defence barrister Andrew Houston said there was "little in the way of mitigation".

Referring to one of the victims Judge Nicholas Rowland told him: "You abused that vulnerable child and threatened her and told her not to tell anyone.

“What you did was to destroy her childhood and what you did will stay with her for the rest of her life.”

In a statement released after the sentencing, DC Hollie Wilson, who was the officer in the case, said: "These were horrific offences and I’m pleased that this sentence will mean that Jacobs is likely to spend the rest of his life behind bars."

The PPO report says Jacobs was rushed to hospital after the alarm was raised by his cellmate at 2.50am on September 27.

Jacobs was given a CT scan, which showed he had suffered a "catastrophic intracranial haemorrhage". He was pronounced dead at 9.04am.

An inquest in September 2023 concluded that he died of natural causes.
